Background check

  • If living, the nominee MUST complete a Background Check using  http://sterlingbackcheck.ca/PGA-of-Canada. Please provide this link to the PGA of Ontario member that you are nominating.  Results will be sent to, Mr. Darcy Kral - PGA of Canada's Privacy Officer.

Career Summary Categories

  • Detailed list of Nominee’s Career Achievements including any support documentation you wish to include. NOTE: The PGA of Ontario reserves the right to retain this material until December 31st, at which point all material is returnable upon request.

Letters of Support (maximum of 6)

  • Letters should provide documentation including dates, names and events that verify the nominee's career and achievements. They should add credibility to the case being presented. Letters can come from PGA members, employers, professors etc. (Current members of the Board of Directors (PGA of Ontario or PGA of Canada) may not write letters of support for a nominee).

All nominations MUST be received on or before 11:59PM EST September 30th of the current year.
